This TDSC® course focuses on the characteristics and specific challenges of terrorist incidents involving mass casualty situations (TerrorMANV), in-hospital triage during highly dynamic scenarios, as well as the surgical treatment principles of Damage Control Surgery (DCS) and Tactical Abbreviated Surgical Care (TASC). In TDSC® courses, we examine in-hospital workflows and processes from both operational and tactical-strategic perspectives.

You will learn decision-making in unfamiliar and complex situations with overwhelmed resources—requiring a change from conventional individualized medical approaches.

Please note that this is a one-day "light" course to bring awareness to the topic and teach the most important dynamics of TDSC®. It does not include a full certifiation.

Unique Characteristics of Terror Incidents

Terror incidents have distinct characteristics and significant differences compared to traditional mass casualty incidents (MANV) or commonly encountered severe injuries (e.g., polytrauma from traffic accidents). These include:

  • Penetrating injuries, especially from gunshots and explosions
  • Critical hemorrhages from extremity and body cavity injuries
  • Complex and dynamic situations, such as second-hit events and unregulated inflows of casualties
  • High importance of self-protection and a robust hospital alarm and emergency response plan
  • Essential cooperation with security authorities in crisis centers and at the scene

The unique features of TerrorMANV often necessitate modifications to established processes. While in conventional medical scenarios “the patient determines the care,” in TerrorMANV, “the situation determines the approach and the care.”

Insights from Military Surgery

Comparable treatment principles are known and proven in military surgery. Colleagues from the Bundeswehr and its Medical Service bring years of practical experience to the course.

You will practice acting and reacting through various dynamic case simulations. These simulations, developed specifically for the TDSC® course using a table-top exercise format, allow small groups to manage scenarios with complex medical and organizational challenges.

A central aspect of the course is to provide you with decision-making tools and practical instruments to enhance your confidence and effectiveness in critical situations.

 

Target Audience

This course is aimed at clinically experienced physicians at the specialist level who take on organizational and surgical responsibilities in their clinics—whether in the emergency department, operating room, intensive care unit, emergency services, or crisis management teams. The TDSC® course builds on expert-level knowledge of individualized medical care and complements it with the tactical and strategic competencies critical in terrorist incidents.

The course is open to all experienced physicians at the specialist level and requires no additional prerequisites. It builds on expert-level medical knowledge and provides the tactical and strategic competencies necessary for handling terrorist incidents effectively.

Course Development and Focus Areas

The TDSC® course was developed in close collaboration between the DGU (including the Working Group for Emergency, Disaster, and Tactical Surgery) and the Bundeswehr Medical Service, with professional support from numerous surgical and military societies.

The course emphasizes in-hospital questions with an operational and tactical-strategic focus. It addresses the unique aspects of terrorist incidents, in-hospital triage in dynamic situations, and the management of DCS and TASC scenarios. Decision-making in unfamiliar scenarios with overwhelmed resources—requiring a shift from conventional individualized medical approaches—is a core focus of the course (tactical surgery). 

Dynamic case simulations, developed specifically for the TDSC® course, train participants to manage complex medical and organizational challenges. In small groups, participants practice responding to demanding scenarios.

This certificate is not valid for certification according to the Weißbuch DGU.
